What the log is for

Ontario long-term care homes have to keep a record of who visited, when, and whom they came to see. This template is a plain sheet for the front entrance with those fields first, laid out to match O. Reg. 246/22 s.267(2), and a few optional columns many homes find useful.

What the regulation says

(2) Every licensee of a long-term care home shall maintain visitor logs for a minimum of 30 days which include, at a minimum,

(a) the name and contact information of the visitor;
(b) the time and date of the visit; and
(c) the name of the resident visited.

The regulation does not say the log has to be electronic, signed, or record a departure time. Those are choices for your own policy.

How to use the template

  1. Fill in the home name and the date range at the top of each sheet.
  2. Keep the sheet at the entrance with a pen. Staff or the visitor fills one row per visit.
  3. When a sheet is full, file it with the date range on the front so it can be found.
  4. The spreadsheet version has the same columns, a frozen header and yes/no drop-downs, if you would rather type.

Keep it out of sight

A paper log lists residents' names, next to the names and contact details of the people who visit them, and everyone who signs it can read the rows above. It should not sit open on a counter. Keep it with staff at the entrance, or on a clipboard that is closed or turned over between visitors, and file full sheets where only staff can reach them.

How long to keep it

At least 30 days. That is a minimum, not a deletion date: keep the sheets longer if your visitor policy, records policy or public health asks you to.
