What it is for

In the United States, where an OSHA standard requires an emergency action plan, OSHA's emergency action plan rule lists among the minimum elements of that plan:

"Procedures to account for all employees after evacuation;" OSHA 29 CFR 1910.38(c)(4), osha.gov

This sheet is a tool for carrying out that kind of procedure at a muster point. It supports your site's emergency action plan; it does not replace it, and it does not decide who should be on the list. In Canada, check your province's requirements for emergency procedures.

Who it is for

Fire and floor wardens, shift supervisors and the safety lead at a single-site plant or warehouse.

How to use it

  1. Before a drill, fill in the names each muster point should account for, from your staff and shift lists.
  2. At the muster point, the warden marks each person accounted for, yes or no, with notes (for example "seen leaving by the north exit").
  3. Bring the current contractor and visitor sign-in sheet, and account for everyone on it who has not signed out.
  4. Record the time all names were accounted for, and report anyone missing to the incident lead and emergency services.
